As stated last month, the prevalence of low back pain (LBP) is REALLY high! In fact, it’s the second most common cause of disability among adults in the United States (US) and a very common reason for lost days at work. The total cost of back pain in the US, including treatment and lost productivity, ranges between $100 billion to $200 billion a year! Is low back pain on the rise, staying the same, or lessening? Let’s take a look!

            In the past two decades, the use of health care services for chronic LBP (that means LBP > 3 months) has substantially increased. When reviewing studies reporting insurance claims information, researchers note a significant increase in the use of spinal injections, surgery, and narcotic prescriptions. There has been an increase in the use of spinal manipulation by chiropractors as well, along with increased physical therapy services and primary care physician driven non-narcotic prescriptions. In general, LBP sufferers who are chronic (vs. acute) are the group using most of these services and incurring the majority of costs. The reported utilization of the above mentioned services was only 3.9% in 1992 compared to 10.2% in 2006, just 11 years later. The question now becomes, why is this? Possible reasons for this increase health care use in chronic LBP sufferers may be: 1) There are simply more people suffering from chronic LBP; 2) More chronic LBP patients are deciding to seek care or treatment where previously they “just accepted and lived with it” and didn’t pursue treatment; or, 3) A combination of these factors. Regardless of which of the above three is most accurate, the most important issue is, what can we do to help chronic back pain sufferers?

            As we’ve discussed in the past, an anti-inflammatory diet, exercise within YOUR personal tolerance level, not smoking, getting enough sleep, and obtaining chiropractic adjustments every two weeks are well documented methods of “controlling” chronic LBP (as there really ISN’T a “cure” in many cases). You may be surprised to hear that maintenance care has good literature support for controlling chronic LBP. In the 8/15/11 issue of SPINE (Vol. 36, No. 18, pp1427-1437), two Medical Doctors (MDs) penned the article, “Does Maintained Spinal Manipulation Therapy for Chronic Nonspecific Low Back Pain Result in Better Long-Term Outcomes?” Here, they took 60 patients with chronic LBP (cLBP) and randomly assigned them into one of three groups: 1) 12 treatments of sham (fake) SMT (spinal manipulation) have over a one month period; 2) 12 treatments, over a one month period but no treatment for the following nine months; or 3) 12 treatments for one month AND then SMT every two weeks for the following nine months. To measure the differences between the three groups, they measured pain, disability, generic health status, and back-specific patient satisfaction at baseline, 1-, 4-, 7-, and 10-month time intervals. They found only the patients in the second and third groups experienced significantly lower pain and disability scores vs. the first group after the first month of treatments (at three times a week). BUT, only the third group showed more improvement at the 10-month evaluation. Also, by the tenth month, the pain and disability scores returned back to nearly the initial baseline/initial level in group two. The authors concluded that, “To obtain long-term benefit, this study suggests maintenance SM after the initial intensive manipulative therapy.” Other studies have reported fewer medical tests, lower costs, fewer doctor visits, less work absenteeism, and a higher quality of life when maintenance chiropractic visits are utilized. The question is, WHEN will insurance companies and general practitioners start RECOMMENDING chiropractic maintenance care for chronic LBP patients?

Low Back Pain Treatment

We have been providing low back pain treatment in San Francisco for over 20 years in the Financial District. We use a combination of chiropractic, deep tissue laser therapy, massage, exercise, and spinal decompression with the DRX9000.

Most of the time spinal decompression is not necessary, but when it is, it’s nice to have onsite.

The most common causes of low back pain are subluxations (misalignment’s) in the lumbar spine, sacrum and pelvis, bulging and herniated lumbar discs, spinal stenosis, degenerative disc disease, and Piriformis Syndrome. There are others but they are rare.

When it comes to low back pain treatment it can be a little tricky. Sometimes patients get better right away and sometimes it takes a while. It depends on the patients age, level of spinal degeneration, any complications, and the nature and severity of the underlying cause of the back pain (which can be difficult to identify).

So, we take all the information we have and formulate a treatment plan. A typical treatment plan at our SF back pain center is 10 visits or so over a 3-4 week period. We will use spinal adjustments, massage, stretching, ice, and home/work modifications to achieve favorable outcomes in the majority of cases. If low back pain or sciatica persists we may want to order an MRI of the lumbar spine to quantify the problem and rule out herniated or bulging disc pathology.

Our next step is spinal decompression with the DRX9000. This is typically 20 sessions over 6-8 weeks. The sessions are one hour long and include muscle stimulation and ice…laser if needed. The cost is $3500. Sure, it’s expensive but we have helped over 2000 patients with the DRX9000 and most are happy they did the treatment. It’s all about quality of life. Arm Pain has many causes from repetitive strain injuries (RSI) to herniated and bulging discs in the neck. If you are someone that spends the majority of your day at a desk in front of a computer (like most of our patients) yo are prone to abnormal wear and tear on the cervical spine (neck), shoulders, arms and hands.

If you have poor posture. either from hereditary predisposition or acquired from poor sleeping habits, ergonomics or injury, you are prone to disc bulges and disc herniations which can lead to pinched nerves in the neck. Pinched nerves in the neck is a primary cause of arm pain.

Arm pain usually presents with either hand pain, shoulder pain, neck pain, numbness in the hands, weakness of grip, or night pain. Arm pain can also present as a stand alone symptom.

The cures for arm pain vary depending on the cause. One thing you should know. Rarely is the cause a “Motrin” deficiency.  When you take a pain killer for arm pain or any type of pain, all you are doing is masking the problem. Pain is a symptom not a condition.

Chiropractic is probably your best bet for arm pain. A chiropractor can examine you to uncover the cause of the arm pain and provide drug-less, nonsurgical solutions on the spot like chiropractic adjustments, massage, ice, and deep tissue laser therapy.

Leg Numbness?

We have been providing nonsurgical treatment for herniated discs in San Francisco for over 20 years. A not so common presentation is that of leg numbness only without low back pain or sciatica. There may or may not be associated leg weakness.

If the leg numbness is new and was brought on by a particular sequence of events such as participating in a  run or walk with a new pair of shoes or doing a new workout or sitting in a new chair for extended periods or even using a new mattress, then there is a good chance you will recover quickly with chiropractic adjustments and therapy.

If your leg numbness is chronic (over 3 months duration) and ongoing, and especially if it is getting worse, you may have a herniated or bulging lumbar disc. This still may respond favorably to conservative chiropractic care or physical therapy.

However, if your leg numbness does not resolve with chiropractic, physical therapy, acupuncture or even medical care, you should probably have an MRI of the lumbar spine to rule out a herniated disc.

If you do have a herniated disc you may be a candidate for nonsurgical spinal decompression on the DRX9000. The DRX9000 is designed and built to treat disc herniations, bulges, stenosis and disc degeneration non-surgically. The treatment is gentle and safe and success rates are high.

Spinal Decompression or Back Surgery?

We have been providing spinal decompression therapy in San Francisco for over 9 years now. The machine we use is the DRX9000. Spinal decompression machines are designed and built to treat disc herniations, disc bulges, disc degeneration, and spinal stenosis non-surgically.

Many of our patients have been told they need back surgery and want to try something less invasive first. Unless your condition is a medical emergency and you risk paralysis without immediate surgery, there is most likely no harm in trying spinal decompression first to see if it may help.

Some back surgeons will scare the heck out of you…especially some of the overseas doctors that are not familiar with the latest advancements with nonsurgical treatments.

If we accept a spinal decompression patient then we feel there is a very good chance of a favorable outcome…and we are usually right. If we are wrong then you are still whole and can pursue other treatment options.

Surgery on the other hand is permanent and cannot be reversed. Even a discectomy,  whereby the herniated disc fragments are removed, alters your structure and can predispose you to problems down the road in the same area or different areas of the spine.  And, the surgery may or may not even make you feel better. The patients that seek us out do not want to take this chance…at least not first.

So, if you have a lumbar disc herniation and are experiencing low back pain, sciatica, leg numbness or tingling, weakness or night pain, you should consider spinal disc decompression. You can always try surgery.

Plus, depending on the type of surgery…if it is a “fusion” surgery, you will forever be permanently disqualified from ever trying nonsurgical spinal decompression.

Do Spinal Decompression Results Last?

We have been providing spinal decompression therapy in downtown San Francisco for 9 years now. During this time we have helped thousands of patients with cervical and lumbar disc herniations, bulging discs, and related disorders, return to more normal lives.

Because we have treated so many cases over an extended period of time, we know that for the most part, spinal decompression results are long lasting.

Depending on the nature and severity of the individual neck or back pain patient’s condition when we started, their overall health and lifestyle, occupation, and initial treatment outcome, we may recommend ongoing maintenance care to improve the odds of the results lasting. Most decompression patients are happy to do this if it means the results will last longer, which they will.

The treatment frequency for maintenance care on the DRX9000 is anywhere from 2 times per month in the beginning to once every couple of months ongoing. In addition, there are exercises, stretches, home care, and nutritional recommendations to help patients maintain results.

Some patients choose to move into chiropractic, yoga, Pilates, or physical therapy for ongoing support. This is good, but none of these do what the spinal decompression machine can do. If you really want the decompression results to last you should probably continue with some sort of maintenance care. You can play around with the frequency until you find an interval that works best for you. Of course, we help you with this process.

DRX9000 spinal decompression

We have been treating disc herniations in San Francisco for over 21 years. About 8 years ago we began to incorporate nonsurgical spinal decompression with the DRX9000 into the mix.

The DRX9000 spinal decompression system is a computerized machine that is designed and built to treat lumbar disc herniations, bulges, stenosis, facet syndrome, and degenerative disc disease without drugs or surgery. The machine we have cost’s over $150,000 because it also treats disc herniations in the neck. There are much cheaper and less effective machines out there so beware.

DRX9000 spinal decompression is the Rolls Royce of spinal decompression systems. We like the machine because it is very comfortable for the patient while at the same time effective. Most of the patients we treat fall asleep during the treatment.

DRX sessions usually last about an hour or less. It takes a few minutes to set you up and harness you so we can connect you to the machine. You are on the machine for 30 minutes or so. Then, we do 10 minutes of electrical muscle stimulation to teach the spinal muscles to work with the spine in it’s new position, and 10 minutes of ice to reduce muscle spasms, pain, and inflammation. Sometimes we will incorporate deep tissue laser therapy if there is a lot of initial pain and inflammation in the beginning.

The DRX9000 treatment protocol is for 20 sessions over a 6 week period. Sometimes it takes more than this. We incorporate exercises at the halfway point. There is extensive home-care instructions including dietary and nutrition recommendations.

Success rates are very high.

Pinched Nerve in the Neck?

Have you been told you have a Pinched Nerve in the Neck? Or, do you just feel like you have one? What is a pinched nerve anyway?

Well, a pinched nerve in the neck is a term used to describe the pressure or trapping of a cervical nerve root mostly from a bulging or herniated cervical disc. Degenerative disc disease, vertebral subluxatons, and facet syndrome can also cause pinched nerves.

Nerves are very delicate and sensitive to any kind of pressure. Famous research studies from the University of Colorado demonstrated that just the weight of a pencil erasure on a nerve can interfere with the transmission of the nerve impulse by 60%.

When nerves become irritated it can cause severe pain and dysfunction. The nerves that exit from the bones in the neck travel down the arm and into the hand. When these nerves become pinched, stretched or irritated it can cause neck pain, shoulder pain, arm pain, and hand pain. Now this does not mean if you have a pinched nerve you will have all these symptoms, heck you might not have any symptoms at first…these are just some of the symptoms that can present from nerve pressure in the neck.

You may also experience numbness and tingling in the arms or hands, weakness of grip, night pain, and swelling in the hands and fingers. Some patients even get headaches.

The symptoms in the hand can mimic those of carpal tunnel syndrome (CTS).  In fact, you can have a herniated or bulging disc in the neck and be experiencing the signs and symptoms of CTS without any neck pain at all. This can lead to a misdiagnoses and unfortunately, unnecessary carpal tunnel surgery.

A chiropractor or orthopedic doctor that specializes in the neck, arms, and hands is your best bet for pinched nerve treatment. They will most likely order cervical x-rays and a cervical MRI…maybe even a nerve conduction test of the upper extremities.

Once a determination and diagnosis is made of the underlying condition, most likely a herniated disc in the neck, then treatment will commence.

Chiropractic care is probably what we would recommend first as it is very safe, gentle and effective. If chiropractic fails to deliver results we can try nonsurgical cervical decompression with the DRX9000c, a machine designed and built to treat pinched nerves resulting from herniated and bulging discs. We would not recommend rushing into neck  surgery unless it is an emergency (most are not). If chiropractic and spinal decompression fail to deliver a favorable outcome you are still whole and can pursue other treatment options, including surgery.
